- Choose your provider and tell us what happened.
- Choose Method of Payment.

- Loss, theft or accidental damage of your device–including liquid and physical damage
- Electrical or mechanical defects after the manufacturer’s warranty has expired
- Accessories, which may include one standard battery, car charger, leather case, home charger and earbud if your device is lost, stolen or damaged.
- Claims limit. For example, depending on your carrier, two claims within any consecutive 12 months with a maximum device value of $1500 per occurrence.
- Deductible: A nonrefundable deductible varies by carrier and depends upon the device model for each claim.
- Cancellation: You may cancel your coverage at any time and receive a pro-rated refund of your monthly fee.
